MassHealt-Connector Cover Sheet -revised Feb 2015

The following link is for a revised cover sheet to be used when faxing or mailing information to MassHealth or the Connector. A separate cover sheet should be printed out for each use (not photocopied) to assure that the bar code works properly.  Using these cover sheets should improve the timeliness & accuracy of document processing for your clients.
