Field Ops Memo 2005-40: Revision ot the TES-EPR-1 Form

When a family is found to be presumptively eligible for shelter placement, this information must be entered on the Emergency Placement Request (TESEPR-1) form prior to submitting the form to Central Office Housing and Homeless Services Unit (H&HS).
