We are forwarding an announcement from MassHealth to enrollment assisters about changes affecting almost 30,000 people in a way that is good but likely to be confusing for those affected. More to come as we learn more.

Group 1: 23,000 people initially selected for renewal between April -June who have not yet successfully renewed will get another chance. 

Background: In June 2021 Sec Becerra wrote to state Medicaid agencies "deeply concerned” with early reports coming in from some states of high rates of “people losing coverage for avoidable reasons that State Medicaid offices have the power to prevent or mitigate” and encouraged states to take advantage of time-limited waiver options to increase the likelihood of successfully renewals based on data-matching (so-called ex parte renewals). MassHealth had asked for and gotten authority to rely on third parties for updated addresses,but had not taken advantage of the waiver options to expand ex parte renewals. Meanwhile, MassHealth was finding that since April 1, its rate of ex parte renewals, which it told us has been about 50% of all cases selected for renewal, was turning out to be much lower. This would mean more paperwork and more procedural denials for people who may be eligible unless the agency took further action.

What happened?

MassHealth has enhanced its auto-renewal process. It doesn’t say how, but two changes they told us about earlier include 1. expanding the reasonable compatibility threshold from 20% to 30%: this means if a renewing member attests to income still under 133% FPL and a data match shows income is higher but by 30% or less, the attestation is verified, and 2. If a renewing member reports zero income or very low income and the IRS match shows no return was filed, the attestation of zero income is verified. We will let you know when we learn more about other enhancements.

What happens next? 

According to the notice 23,000 households who had been sent blue envelopes and not yet successfully renewed will be re-selected in June and July. Once re-selected MassHealth will try again to see if individuals in these households can be auto-renewed under the enhanced process. If so, they will get a white envelope confirming their eligibility. If not, they will get another blue envelope and another opportunity to renew.

Group 2: 5,000 people subject to an asset test in the MA-21 system who were terminated after April 1 have been reinstated and sent a new renewal form.

What happened?

According to the notice, 5,000 people in the MA-21 system (this is the system for people eligible as 65 or older or needing long term care) who had been protected and selected for renewal were terminated for non-response without an adequate termination notice. This happened because of a defect in the system that has now been corrected. They have now been reinstated and sent a blue envelope to renew. That’s all the information we have to date.

Earlier, MassHealth had identified “inadvertent” terminations occurring after April 1 for people who had turned 65 but were still “protected” in the HIX system who would once again be “protected” for 90 days during the unwinding. At the same time, we’ve been reporting to senior Medicaid officials other cases of no-notice terminations, terminations after April 1 based on renewals initiated before April 1, and other erroneous terminations during the unwinding. We don’t yet know whether the system fix and reinstatements relate to one or more of these known issues or something new.

MLRI has prepared the attached flyer to help advocates identify terminations that are erroneous under the timing rules in effect during the unwinding period from April 1, 2023-March 31, 2024. It includes tips for individual advocacy to help clients get reinstated, and citations to the applicable state and federal authorities. 23,000 MassHealth Households Reselected for Renewals in June & July 2023

In June and July 2023, MassHealth reselected ~18K households in the HIX system and ~5K households in the MA21 system for renewal. These 23,000 households have now been picked up for renewal a second time; learn below what you need to know and how to support members.

 

Why is MassHealth restarting renewals for 18,000 households in the HIX System?

 

MassHealth is committed to minimizing the number of members who are terminated for procedural reasons. As part of this commitment, MassHealth has recently implemented several new enhancements to its auto-renewal processes. Around 18,000 households have been identified to have their renewal restarted in the hopes that some of these households will be auto-renewed now that the new enhancements are in place. These households were selected if they met the following criteria:

  • Were selected for renewal between April and June 2023
  • Were not successfully auto-renewed during their initial renewal
  • May be auto-renewed now that new auto-renewal improvements are in place.

 

Why is this important?

 

These households were reselected for renewal in June and July of 2023. Households in this group who are still not successfully auto-renewed will be sent another blue envelope with a new due date to return their renewal in August or September. This means that you may start to hear from members who have received two blue envelopes within a short period of time. Note that HIX (the Assister Portal and Individual Portal) will not be able to identify these 18K households from other households selected for renewal.

 

How to support these Members?

 

Please instruct them that if they receive a blue envelope, it always means that they need to complete and return the enclosed renewal.

 

Why is MassHealth restarting renewals for 5,000 households in MA21?

 

A systems issue in MA21 related to renewing certain households that had their eligibility protected was recently identified. About 5K households in the impacted population were picked up for renewal and have been terminated for non-response since April 1, 2023. MassHealth has identified that these individuals received insufficient notice of termination; the system issue has now been corrected. These impacted ~5K households have had their coverage reinstated and were sent a new renewal packet to complete.

 

You may start to hear from these members who have also received two blue envelopes within a short period of time. The new due dates for this group will be at the end of August or the beginning of September.

 

How to support these Members?

 

You should instruct them that if they have received a second blue envelope, it means that they need to complete their renewal.

 

Additional important information about the impact of restarting these renewals (applicable to 18K HIX households only):

 

1) What are the enhancements to the auto-renewal process for HIX households that MassHealth has made?

 

MassHealth’s auto-renewal process now includes the following enhancements:

  • Using additional data sources to supplement missing information including federal IRS data, state DOR data, and others
  • Using SNAP income data to auto-renew members eligibility
  • Conducting “Express Lane” renewals for children, allowing MassHealth to use data from several sources to streamline renewal processes
  • Increasing premium factor from $50 to $100, meaning that blue renewal envelopes will only be sent if the premium amount increases by $100+ rather than $50+
  • Auto-renewing all individuals with $0 income
  • Using federal and state income data for members whose last renewal did not include income data (i.e., null income)

 

2) Did any of the 18K households in HIX have their coverage terminated?

 

None of the impacted members were terminated or received a lesser benefit.



3) Will Assisters be able to identify using MassHealth’s systems (HIX or MMIS) the impacted 18K households?

 

No, there will be no way to distinguish between the 18K households who were reselected to receive another renewal packet. It’s important to help members understand they need to complete their renewal if they receive a blue envelope.

 

4) What is the new expected timeline for when households that had their renewal restarted to return their renewal to MassHealth?

 

Households in this group will have their renewal deadline at the end of August and beginning of September.

 

5) What due date will show in the portal for households in this group?

 

Households in this group that are not auto-renewed will get a new due date. This new due date will appear in their account on their renewal banner. Note that this renewal banner will be the standard renewal banner, it will just have an updated due date.

 

6) What renewal/eligibility status will show in the portal for households in this group?

 

Once a household has been re-selected for renewal, the HIX will show that their application is under renewal with an active deadline identifying when the renewal is due. The renewal banner with this information will be displayed at the top of the My Account Dashboard screen informing the member that they've been selected for renewal. The due date on the banner will be their new due date (in August or September). These members will not be closed or terminated until or unless they don't respond by the due date.

 

7) Will the blue envelope renewal packages that the 18K households receive have any special messaging or inserts in them?

 

No, there is not a special insert for these selected members. However, all blue renewal packets have informational inserts about the renewal process and resources on how to submit a renewal.
