Collateral Contact Form (Voluntary Authorization to Release Information/ VARI)

This form (VARI-OI) allows DTA worker to assist client with obtaining verifications from third parties such as employers, landlords, schools, etc.  Complete and send this form to DTA for verifications you do not have that are required for your SNAP benefits.  DTA also has a separate from for employment verification, DOR-L  form that can be used to contact employers. Either voluntary authorization form should be sufficient, or a hand written statement from your client giving DTA permission to get infomraiton from a third party. 

DTA cannot require one specific form in order to do a collateral contact. You can write a letter or a note or you can fill out this form
